Information collected
When you join the pre-launch waitlist, MigdaMD may collect your name, email address, optional phone number, city or ZIP code, preference for in-person or virtual care, how you heard about the practice, and the date and time of your consent.
When you use the general contact form, MigdaMD may collect your name, email address, optional phone number, question topic, message, and the date and time of your consent. The contact form is intended only for general practice questions and should not include medical information.
The website's hosting and security providers may also process limited technical information, such as IP address, browser type, device information, and basic activity logs needed to operate and protect the website.
How information is used
MigdaMD may use website-form information to manage launch interest, provide opening and membership updates, understand general geographic demand and care-format preferences, respond to general questions, and maintain the security and reliability of the website.
How information may be shared
MigdaMD does not currently sell or rent website-form information. Information may be processed by service providers that support website hosting, data storage, communications, and security. MigdaMD intends to retain waitlist and contact-form information only as reasonably necessary for communications, practice planning, legal obligations, and legitimate operational needs. Reasonable administrative and technical safeguards will be used, but no website or electronic storage method can be guaranteed to be completely secure.
Your choices
You may unsubscribe from launch communications or request that your website-form information be corrected or deleted by contacting hello@migdamd.com. Some information may be retained when required by law or for legitimate recordkeeping and security purposes.
Children's privacy
The website and pre-launch waitlist are intended for adults and are not directed to children under 18.
Policy updates
This policy may be updated as MigdaMD finalizes its technology, vendors, enrollment process, and clinical operations. The revised date will appear at the top of this page.
